  • About the role

  • Our Hospital Social Work Team provides timely Social Care Act assessments for people following hospital discharge who may have ongoing social care needs.

    We support adults aged 18 and over, including those with complex circumstances, whether they are new to the Local Authority or require continued care.

    This includes arranging community care packages, placements, step-down beds and making Best Interest decisions where required.
     
    As a Newly Qualified Social Worker (NQSW) in this team, you will:
    • Manage a supervised caseload, including safeguarding adults.
    • Accept referrals from your Supervisor or Team Manager.
    • Carry out interviews and assessments in hospitals, care settings or people’s homes.
    • Identify key issues, set objectives and take effective action to achieve solutions.
    • Use a variety of social work models and techniques to address client needs.
    • Apply resources appropriately to deliver positive outcomes.
    • Maintain confidentiality and compliance with data protection and safeguarding standards.
      Receive regular supervision and develop reflective practice.
    •  Work collaboratively with council colleagues, health professionals, the police, voluntary organisations and other partners.
    The team operates Monday to Friday, 9:00 AM to 5:00 PM, from the Civic Centre in Uxbridge.

    Out-of-hours support is provided by the Emergency Duty Team. 

    We are looking for a Newly Qualified Social Worker (NQSW) who is motivated, organised and committed to supporting residents and their families.

    To succeed in this role, you must have:
    • A recognised Social Work qualification and registration with Social Work England.
    • Commitment to professional registration and compliance with DBS requirements.
    • Willingness to participate in and complete the Assessed Year in Employment (ASYE) programme.
    • A proactive, team-oriented approach, with the ability to manage your own workload.
    • Strong organisational skills to balance competing priorities.
    • An understanding of the Social Work Professional Capabilities Framework (ASYE level).
    • A reflective approach to practice, recognising your own limits and seeking guidance from supervisors or experienced colleagues when required.
