  • About the role

  • We are looking for a dedicated and compassionate Community Care Worker to join our Adult Social Care team. This is a key frontline role supporting vulnerable adults across the borough, helping them to live as independently as possible while ensuring safe, effective and timely support. 

    The Community Care Worker supports vulnerable adults by carrying out assessments, care planning and reviews in line with the Care Act and local policies. The role focuses on promoting independence, identifying eligible needs and ensuring individuals receive appropriate, person-centred support.

    Key responsibilities include:

    • Managing a caseload and undertaking assessments to identify eligible needs
    • Developing and reviewing care plans to ensure support remains appropriate
    • Promoting independence and supporting individuals to achieve positive outcomes
    • Working within the statutory framework for Adult Social Care, following legislation, guidance and local procedures
    • Working collaboratively with health professionals and partner agencies
    • Supporting safeguarding activity to protect adults at risk
    • Maintaining accurate records and ensuring compliance with practice standards

    This role involves regular contact with service users, carers and professionals, requiring strong communication, organisation and a proactive approach.
