  • We are seeking a dedicated and high-performing Adults’ Social Worker who is passionate about supporting individuals through critical life transitions.     You will join our Preparation for Adulthood & Autism Team, supporting young people transitioning from children’s to adult services and responding to autism referrals.

    Key Responsibilities

    • Work collaboratively within the Transition Service and alongside the Children with Disabilities Team, Leaving Care Team, and Special Educational Needs (SEN) services.

    • Undertake timely and responsive assessments, re-assessments and reviews to deliver personalised care that promotes maximum independence and safety.

    • Deliver comprehensive case management, including Education, Health and Care Plans (EHCPs), health action planning, and person-centred planning.

    • Participate in Continuing Health Care (CHC) assessments where appropriate.

    • Support young people in their transition into employment.

    • Build effective partnerships with statutory and voluntary organisations to ensure holistic support for service users.

    • Contribute to team development and represent the service when required.

    • Apply the Mental Capacity Act, including undertaking Best Interest decisions and Community Deprivation of Liberty Orders (DoLs).

    • Make referrals to supported living and manage safeguarding enquiries.

    • Carry out risk assessments and develop protection plans for service users.

    About You

    We are looking for a qualified Social Worker with a strong understanding of both children’s and adults’ legislation.

    You will have:

    • Experience working in a transitional role.

    • A passion for supporting young people and adults with autism.

    • A collaborative, proactive approach to practice.

    • Proven active field work experience within Preparation for Adulthood transitions, including experience of SEND.

    Please note, this role is for an experienced Social Worker and is not suitable for ASYE.

    Working Arrangements

    This is a hybrid role; however, officers are required to complete visits when not office-based.

    • The duty role requires officers to be in the office from 9am to 5pm on a rota system.

    • Duty can be more than one day per week.

    • Officers are required to be available to attend the office at any time if required.

    This is a busy and fast-paced team, and proven field work experience is essential to ensure the needs of the service are met.
