  • About the role

  • We are looking for a high-performing and experienced Adults’ Social Worker to join our innovative and forward-thinking local authority. This is a fantastic opportunity for someone seeking to progress their career in a supportive environment that values development, collaboration, and person-centred practice.

    We currently have a vacancy in our Preparation for Adulthood & Autism Team, which supports young people transitioning into adult services and manages autism referrals. This dynamic role offers the opportunity to work closely with:

    • The Transition Service
    • Children with Disabilities Team
    • Leaving Care Team
    • Special Educational Needs (SEN) services

    Key Responsibilities

    • Carry out timely and responsive assessments, re-assessments, and reviews to deliver personalised care and support to service users and carers.
    • Promote independence, safety, and wellbeing through person-centred planning.
    • Case management, EHCPs, Health Action Planning, and duty management within a fast-paced environment.
    • Manage safeguarding referrals under Section 42 and Community Deprivation of Liberty Safeguards (DoLS).
    • Undertake Mental Capacity Act assessments, Best Interest decisions, and Community DoLS applications.
    • Participate in Continuing Health Care (CHC) assessments.
    • Support young people in accessing employment opportunities.
    • Liaise effectively with statutory bodies, voluntary organisations, and other partner agencies.
    • Make referrals to supported living services and develop risk assessments and protection plans.
    • Contribute to service development, represent the team when required, and share best practices across the service.

    What We’re Looking For

    • An experienced Adults’ Social Worker with a strong understanding of transition and autism pathways.
    • A proactive and organised individual with a commitment to improving outcomes for young people.
    • Strong knowledge of relevant legislation, including the Care Act, Mental Capacity Act, and safeguarding protocols.
