  • About the role

  • We have a great opportunity for an experienced paid Carer or Support Worker with knowledge of working in adult statutory services who is looking to move into a senior or supervisory role.

    As a Team Leader, you will be responsible to the Registered Care Manager for the day-to-day management of the care and support of all service users. You will provide leadership, guidance and professional standards to staff, ensuring high-quality, person-centred care is consistently delivered.

    Working closely with the Registered Care Manager and other professionals within the Social Care and Health system, you will help identify and deliver user-focused support plans that enable people to live full and active lives.

    This is a pivotal role within a large care home supporting adults with learning disabilities. You will ensure the people we support have maximum opportunity to develop independence, skills and positive outcomes, while meeting their day-to-day needs.

    Key Responsibilities

    • Support the Registered Care Manager in the day-to-day running of the service and deputise when required.

    • Supervise and support designated key workers, ensuring professional standards are maintained.

    • Ensure regular reviews of service users take place and that confidential records are accurate and up to date.

    • Ensure service users have designated link/key workers and maintain links with families, advocates and friends.

    • Ensure care and support plans are implemented effectively and individual needs are met.

    • Organise and embed person-centred support planning to help individuals achieve their goals and ambitions.

    • Plan and arrange social and recreational activities, including holidays where appropriate.

    • Administer medication in line with LBH Adult Resources medication policy.

    • Ensure compliance with Health and Safety requirements, undertake risk assessments and monitor hazards.

    • Maintain professional relationships that respect the dignity of service users at all times.

    • Support safeguarding, promote independence and act to prevent harm.

    • Ensure rotas and routines meet the needs of service users.

    • Attend and contribute to staff meetings, supervision and appraisals.

    • Work collaboratively with internal teams and external agencies to deliver coordinated support.

    About You

    You will:

    • Have previous experience working in adult statutory services.

    • Have experience supervising others in a paid or voluntary capacity.

    • Be confident providing leadership, guidance and professional support to a staff team.

    • Be willing to provide personal care.

    • Have strong ICT skills and experience using systems for record keeping and care management.

    • Understand person-centred planning and be committed to supporting independence and positive outcomes.

    • Demonstrate awareness of safeguarding, health and safety and professional care standards.

    • Be organised, accountable and able to work proactively in a busy environment.
